Request Letter. This is a formal letter requesting the donation of goods or services. Print this out and take it to businesses to ask if they will donate to the Medical Clinic. Click Here.
Community Group Involvement
Volunteer. Community Group Leaders and Members, encourage everyone in your group to Sign Up to Serve! Children are welcomed and encouraged to serve with their parents, so that they may see how the body of Christ, the Church, truly cares for people.
Give. As a group, collect items amongst yourselves, and consider involving your neighbors! Walk your neighborhood and ask people to donate goods for the upcoming clinic. Tell people that you will return next week to gather items! Click here for a list of Community Group Donation Needs.
Recruit Professionals. Enlist your Community Group to make a list of everyone’s Doctors and Dentists. Print off the Medical Professional Letter, Medical Clinic FAQ, and Medical Clinic Forms (linked in the section below) and take them to the offices that are on your list. Personally invite your Doctors, Dentists, Chiropractors, Massage Therapists, and more! Click here to download the Medical Professional Packet.
Promote. Pass out Flyers in Germantown, Shelby Park, and Schnitzelburg as a group during the week of Feb. 6-9.
Medical Professionals
We are in need of licensed medical professionals to serve at the clinic. If you are a medical professional we welcome you and invite you to serve at Sojourn’s Free Medical Clinic. It has been a life changing experience for some of our volunteers. In our Medical Professional Packet, you’ll get the following:
• A Medical Professional Letter, which gives an explanation of our Medical Clinic.
• An FAQ with our most common inquiries.
• A sample medical form used for triage, exams, screening results, and follow up information.
• An official statement about your up-to-date medical license to be signed and notarized before the clinic. You may mail the statement to the Sojourn offices (930 Mary St., Louisville, KY 40204) or bring it with you the day of the clinic. Note: The statement can be notarized the day of the clinic as long as you bring a copy of your license.
