On FEBRUARY 25, 2012, Sojourn’s Midtown campus (located at 930 Mary Street in the Germantown neighborhood) will be transformed into a medical facility featuring a wide variety of health care services. The clinic opens at noon, and we’ll accept the last patient at 4 p.m. There are a number of ways to be involved:
SERVE
PROVIDE MEDICAL SERVICES – Doctors, Nurses, Dentists, and other practitioners are needed to help provide services. There is a triage area where nurses will be doing general inquiry and check-ups: blood pressure, glucose readings, etc. A separate area for doctors’ offices is available for more in-depth care of our guests. Certified health professionals only, please.
To download a copy of our informational doctors packet, click HERE. Included is a Medical Professional Info Letter, which gives an explanation of our clinic; a document answering our most frequently asked questions; sample medical forms we use for triage, exams, screening results and follow-up information; and an official statement about your up-to-date medical license to be signed and notarized before the clinic. You may mail the statement to the Sojourn offices (930 Mary St., Louisville, KY 40204) or bring it with you the day of the clinic. Note: The statement can be notarized the day of the clinic as long as you bring a copy of your license. If you have further questions, please contact Clinic coordinator Kathy Chavez at kitkat1824@hotmail.com.
BE A FAMILY ADVOCATE – Advocates accompany families through the process during the day, encouraging them and giving support and comfort when it’s needed most. This is a wonderful opportunity to establish lasting friendships with those in the neighborhood. (If you’re not a Sojourn attender, please contact Josh Thomas at jthomas@sojournchurch.com before signing up to become an advocate)
ASSIST WITH MEDICAL FORMS – Upon our guests’ arrival, they will then need to fill out necessary medical forms before they see a nurse or doctor. Help our guests fill out these important forms.
THE SOJOURN SALON/SPA – We offer haircuts and other salon services from certified cosmetologists and nail technicians. This is one of our most popular areas. Licensed professionals only, please.
SERVE A WARM MEAL – A warm meal is always included at a Sojourn Clinic. Help facilitate hospitality for our guests in the Family Dining Room. Work the grill or serve food and beverages. We don’t want any hungry stomachs to leave our clinic.
WORK THE FAMILY CARE AREA – This room will be available for all of our guests to peruse. They will be able to get any needed groceries, toiletries, clothing, or shoes.

DONATE
Though we’d love to see you on Feb. 25, you don’t have to be present to serve our clinic. An undertaking like this requires a huge amount of supplies and donations. Click HERE for a donation list for everything from medical supplies to hygiene to salon items. If you’d like to solicit donations for our clinic from businesses, a formal request letter can be found HERE.
ADOPT
This unique opportunity is available to Sojourn’s community groups. Take more ownership in the weekend by being responsible – under the guidance of a clinic leader – for one particular area of the clinic. To download an informational one-sheet about what this entails, please click HERE.
